Become a Client

Starting your lab services with EMLab P&K is easy and only takes a few moments:

New Client Account Set-Up

Step 1: Print and complete the New Client Enrollment Form (pdf)

Please Note:

Our standard credit terms are Net 30 for approved credit accounts, and a standby credit card authorization is needed on file.

First orders for new clients need to be prepaid by either credit card authorization OR prepaid by check.

Credit card or other prepayment allows us to begin your lab services immediately, while we process your credit application.

Minimum orders for established accounts are $100.

Step 2: Fax your completed form to us at (800) 444-8156.

Step 3: Mail or deliver your samples to us for analysis (see below for details).

Submitting Samples for Analysis

Step 1: Complete a Chain of Custody form for each shipment of samples.

Step 2: Ensure that your account is set up or prepayment has been arranged.

Step 3: Mail or deliver your samples and chain of custody form.

Option #1: Submit sample information online through LabServe™.

Option #2: Download and complete our Chain of Custody form (pdf). Retain a copy for your records.

Option #3: Use your own Chain of Custody form
